Register for the 2006 ILL Forum

Please note the following:

• VIVA funds will reimburse both the public and private nonprofit members for their travel to the ILL Forum.
• If you must stay the night, we will need a Travel Authorization form before the Forum. Please FAX it to Liz Wright at (703) 993-4662.
• Make sure you request the conference rate from the hotel.
• Take a moment to review VIVA's Travel Memo for more reimbursement information.

To register, please send an email to the VIVA Office with the following information:

• Your Name
• Your Institution,
• Your Email address
• And whether or not you need a Vegetarian Lunch.
