Standard Forms

The following standard forms have been provided by Iris Moubray in the JMU Procurement Office, which handles procurement for VIVA. These forms are posted for the convenience of VIVA members.

COMMONWEALTH OF VIRGINIA AGENCY CONTRACT FORM ADDENDUM TO CONTRACTOR'S FORM (for use with all vendor contracts)

Statement to be used on the agreements when the addendum form is part of the agreement.

 

User Rights and Responsibilities

Contracts for most current VIVA resources are provided here (in Adobe Acrobat PDF and/or Word form) to help our members know the rights and responsibilities associated with each VIVA license. Many key contract provisions are not included here, but would be included in accompanying documentation (e.g. RFPs, follow up letters, and email) in the file. Please contact the VIVA Procurement Office if you have specific questions. The Contract Numbers are included for most.
